You might hear advice like “exercise reduces migraine frequency,” yet you may also recall a specific workout that seemed to launch you straight into an attack.

This contradiction is real. In fact, Migraine Buddy user data highlights this paradox perfectly.

  • 3.6% of users report “Exerciseas a trigger, and 3% report “Physical exertion.”
  • However, 2.3% of users also list “Exerciseas a relief method.

While these numbers in the app might seem low (often because users must manually add these specific tags), external research paints a broader picture. A large U.S. cohort study found that 22% of patients reported exercise as a trigger [1], while a Dutch clinic-based study found the lifetime prevalence to be as high as 38% [2].

So, is exercise a friend or a foe? The answer is often “both,” depending on when and how you move. To help you navigate this, we are introducing the Traffic Light System. It’s a simple framework promoted by experts to help you decide when to push, when to pause, and when to stop.

Why Exercise Feels Unpredictable in Migraine

If you have ever hesitated to put on your running shoes because you were afraid of a headache, you are not alone. This unpredictability leads to two common questions from the Migraine Buddy community:

  1. “Can exercise worsen a migraine attack?”
  2. “Can exercise actually help me through an attack?”

The biology of migraine involves a sensitive brain that craves homeostasis (stability). Sudden changes, like a spike in heart rate or body temperature from strenuous exercise, can sometimes overwhelm this stability, acting as a trigger [3]. Conversely, regular movement releases endorphins and reduces stress, which stabilizes the brain over time and raises the threshold for attacks [4]. This dual nature is why a “one-size-fits-all” rule doesn’t work.

The Traffic Light System vs. Migraine Attack Phases

To answer the pressing questions “Is it safe to exercise during prodrome? How do I know if I should stop?”, we can map your physical activity to the phases of a migraine attack using a Traffic Light System.

🟢 1. Green Light: The Interictal Phase (Between Attacks)

When movement can be helpful

The “Green Light” is your baseline state: the days when you are migraine-free.

Large population-based studies link higher habitual physical activity to lower migraine prevalence [5]. Regular, moderate aerobic exercise has been shown to reduce mean migraine days by about 0.6 days per month [6].

Why does it help? Think of this as “conditioning” your nervous system. Regular, moderate movement (like brisk walking, swimming, or cycling) raises your threshold for triggers, making your brain more resilient against future attacks [4].

🟡 2. Yellow Light: The Prodrome Phase

Proceed with caution

This is the pre-attack phase, occurring hours or days before pain hits.

30% of Migraine Buddy users report weakness during the prodrome phase. This makes the Yellow Light tricky.

So, how to distinguish normal exercise fatigue from migraine-related weakness?

  • Normal fatigue: Your muscles feel tired after exertion, but you feel mentally clear or energized.
  • Migraine weakness: You feel a “neurological drain” (heavy limbs, yawning, or brain fog) before you even finish (or start) the workout.

If you are in the Yellow zone, you don’t necessarily have to stop, but you must shift gears. Swap high-intensity training for gentle yoga or stretching. The goal here is maintenance, not performance.

🔴 3. Red Light: The Attack Phase

Stop and rest

Once the attack has started, the light is Red.

75% of Migraine Buddy users report their pain feels worse if moving (“worse pain if moving” in the app). During an attack, the neurons are hypersensitive. Even the thud of your footsteps or the rhythmic movement of your head can translate into throbbing pain [3].

Red Light is not the time to push through. Respect the Red Light to prevent the attack from escalating in intensity and duration.

The Exercise Paradox in Migraine

Why can exercise be both a trigger and a relief?

Intensity Matters

Strenuous exercise is a known trigger for some, likely due to sudden dilation of blood vessels or exertion-induced pressure [1]. However, moderate exercise usually acts as a protective factor.

The Relief Mechanism

For a small subset of users (2.3% in Migraine Buddy), light exercise during the very early onset might abort an attack by regulating blood flow or reducing muscle tension, but this is highly individual.

What type of exercise may trigger an attack

There is no universal list, but “exertional headaches” are distinct. If your pain comes on immediately during peak exertion (like lifting a heavy weight) and fades when you stop, that is an exertional headache [7]. If the migraine slowly creeps in hours after a workout, it may be a “delayed trigger” caused by dehydration or a drop in blood sugar post-exercise.

Myth-Busting

Let’s clear up some common misconceptions that might be holding you back.

❌ Myth: “Exercise is bad for migraine.”

✅ Fact: A sedentary lifestyle is actually associated with higher migraine frequency. Movement can be a relief, provided the intensity and timing is right [5].

 

❌ Myth: “If I triggered an attack once, I should stop exercising forever.”

✅ Fact: Triggered once ≠ always a trigger. Factors like hydration, sleep, and stress levels on that specific day likely played a role.

 

❌ Myth: “Avoiding movement prevents pain.”

✅ Fact: Strict avoidance can lead to “deconditioning,” where your threshold for physical stress drops even lower, making you more sensitive to movement in the long run [4].

 

When to Talk to a Doctor

While most exercise-induced headaches are benign, it is vital to track your patterns. If you notice a clear link between exertion and pain, bring your migraine records to your doctor.

Red Flags that require medical attention:

  • Head pain that strikes suddenly like a “thunderclap” during exercise.
  • New exertional headaches, if you’re over 50.
  • Headaches associated with loss of consciousness or vomiting during exercise.

Tracking helps you and your specialist differentiate between a migraine triggered by exercise and a primary exertional headache, as the treatments for these can differ.

Bottom line

Exercise and migraine have a complex relationship, but you don’t have to choose between exercising and being pain-free.
